<p>High Deductible Health Plans are medical insurance polcies that don&#8217;t cover first dollar medical expenses, other than routine care. They can also be a:<br />
&lt;ul&gt;<br />
&lt;li&gt;• Health Maintenance Organization&lt;/li&gt;<br />
&lt;li&gt;• PPO&lt;/li&gt;<br />
&lt;li&gt;• Indemnity plan&lt;/li&gt;<br />
&lt;/ul&gt;<br />
Health Savings Accounts were created by the December 8, 2003 Medicare legislation that was signed into law by President Bush. These accounts are modeled after Archer MSAs.</p>
<p>&lt;h3&gt;Individuals who are eligible for HSAs include those that are:&lt;/h3&gt;<br />
&lt;ul&gt;<br />
&lt;li&gt;• Covered by an HDHP&lt;/li&gt;<br />
&lt;li&gt;• Not covered by medical insurance &lt;/li&gt;<br />
&lt;li&gt;• Not enrolled in Medicare&lt;/li&gt;<br />
&lt;li&gt;• Can’t be claimed as a dependent on someone else’s tax return&lt;/li&gt;<br />
&lt;/ul&gt;<br />
There are no income limits that contribute to HSAs and people aren&#8217;t required to have earned an income to contribute to an HSA.</p>
<p>There are certain types of medical benefits that&#8217;ll make you ineligible for an HAS. These are typically referred to as “1st dollar” medical benefits, such as:<br />
&lt;ul&gt;<br />
&lt;li&gt;• Medicare&lt;/li&gt;<br />
&lt;li&gt;• Flexible Spending Arrangements&lt;/li&gt;<br />
&lt;li&gt;• Health Reimbursement Arrangements&lt;/li&gt;<br />
&lt;li&gt;• Tricare Coverage&lt;/li&gt;<br />
&lt;/ul&gt;<br />
A high deductible plan is a medical insurance with a minimum deductible that is $1,100 for individual coverage and $2,200 for family coverage. Annual out-of-pocket is limited and includes deductible as well as co-pays and are set at $5,500 for individuals and $11,000 for family coverage. All covered benefits in a plan must apply to the plan deductible and include prescription drugs.</p>
<p>If HDHP provides prescription drug benefits, then the prescription drug expenses must be subject to a deductible or the individual may not contribute to the Health Savings Account.</p>
<p>In a high deductible plan, routine care doesn&#8217;t include any service or benefit that is intended to treat an illness, condition or injury that is already in existence. There are certain drugs and medications that can be considered routine care. These drugs are drugs such as cholesterol-lowering medication for individuals who are suffering from high-cholesterol.</p>
<p>Contributions to a HAS can be made by either the employer or individual and both. If the contributions are made by the employer, the amount isn&#8217;t taxable. If the contribution is made by an individual, the amount is considered an “above the line” deduction.</p>
<p>If others make contributions on behalf of the individual, these contributions can be deducted by the individual as well. As of 2007, individuals are allowed a one-time transfer from their IRA to an HAS as well. There are maximums that are set at $2,850 for self-only coverage and $5,650 for family coverage. Once an individual is enrolled in any kind of Medicare, they can&#8217;t receive contributions to their account.</p>
<p>Although there are numerous benefits to Health Savings Accounts, there are also several drawbacks. The main drawback is that you must have your deductible paid before you can receive benefits from your medical insurance policy. Although these accounts pay for your basic routine care, there are certain areas afterwards that may not be covered.</p>
<p>These plans often tend to benefit only two groups of people, those that are very healthy and those that are very ill. That is because you typically don&#8217;t need to pay for numerous medical expenses. At the same time, those who are very ill and do have large medical expenses on a monthly basis. Thus, once your deductible is met, the plan will pay for medications with the same co-pay as your other medical expenses.</p>

<p>Recently, I learned just how challenging enrolling for private medical insurance can be in the  United States. I was going off my group medical insurance and needed private coverage. <p>That was the easy part.</p>
<p>A week after completing the enrollment form, I got a follow up call to my enrollment form from the &#8220;medical department&#8221; of the company. I was told this was to verify some of the information on my enrollment form.</p>
<p>And here&#8217;s where things got a bit rough.</p>
<p>The woman who interviewed me wanted VERY detailed information on medical claims. Since the specific details she wanted were in my doctor&#8217;s medical records, not at home, I was unprepared for her questions. I made every effort to be helpful, but the call rapidly became an unfriendly interrogation.</p>
<p>For almost an hour, I was asked the same questions over and over until I felt like shouting, &#8220;Asked and answered!&#8221; For most of them I simply replied &#8220;I don&#8217;t know.&#8221; Eventually I asked her to contact my physicians. When she finally did, they checked my records and answered her questions.</p>
<p>By the time I hung up the phone, I felt like I&#8217;d gone three rounds with Mike Tyson. I was both frustrated that I couldn&#8217;t answer her questions and emotionally exhausted. She treated me like a criminal instead of a person seeking private medical insurance. It wasn&#8217;t a good experience!</p>
<p>Because of what happened to me, I did some research. It helped me to compile the following list of ten things I believe anyone enrolling for private medical insurance in the United States should know. I hope this list will help you or someone you know to avoid what happened to me.</p>
<p><strong>Before You Apply For Private Health Insuarnce</strong></p>
<ol>
<li>Do not let your policy expire prior to seeking new coverage. If your policy is being canceled you can go on the Consolidated Omnibus Budget Reconciliation Act (COBRA). This act ensures continuation of group health coverage that otherwise might be terminated.</li>
<li>Talk to your physicians about your medical records before you apply. For private insurance, companies can review your medical history as far back as ten years. Therefore, ensure your records are up-to-date and talk with your physician(s) about any health issues that might be red flagged.</li>
<li>Buy copies of your medical records. You can also request a copy of your medical records under the Health Insurance Portability and Accountability Act (HIPAA). For more information on HIPAA, go to http://www.privacyrights.org/fs/fs8-med.htm</li>

<p><strong>If You Get A Follow Up Call&#8230;</strong></li>
<li>Answer only the questions asked. Do not volunteer any additional information. Be courteous and remember the interviewer isn&#8217;t your friend or your medical doctor. In these kinds of interviews you have NO privacy protection. In addition, the interview will most likely be recorded so think before you respond.</li>
<li>If you don&#8217;t know the answer to a question, be honest about it. I recommend you get comfortable with saying, &#8220;I don&#8217;t have that information, please contact my doctor.&#8221; When asked specific questions about medical procedures don&#8217;t guess, refer them to your doctor(s) and your medical records.</li>
<li>Be honest and stick to the facts regarding your medical history. The business you are enrolling to may have access to your MIB file, and already know certain aspects of your medical history. If you are caught lying, you can be denied. Most enrollment forms ask if you have ever been denied medical insurance, and a denial can be an immediate &#8220;red flag&#8221; on any future enrollment forms.</li>
</ol>

			<content:encoded><![CDATA[<p>A significant number of Americans are insured under group health plans and the rules governing group plans in the state of Florida are similar to those seen in many other states, although there are several differences which might apply for public employees.<br />
To join a group <strong>health plan</strong> you must first be eligible for memberships of the scheme. For instance, while an employer may operate a group health scheme, it might not be open to everybody, perhaps being aimed at only full-time workers. Additionally, the scheme may be run by a Health Management Organization (HMO) and you may find that you are living outside of the HMO&#8217;s service area.<br />
Assuming that you are eligible to participate in the scheme then you have to be allowed to join whatever your state of health. For this purpose your state of health refers to your current health, taking into account any disability which you may have, as well as your previous medical history. It should also be noted that you cannot be excluded on the basis of genetic information.<br />
It must be understood here that, while your employer is allowed to exclude you from a scheme because you do not for instance work enough hours, he is not permitted to exclude you based solely on your current or prior medical history.<br />
Most plans has an enrollment period during which you must elect to join the scheme which may typically be within about 30 days or joining the company. However, if you decide not to join at this stage then your employer must give you the opportunity to join during what is often called a special enrollment period where particular changes take place within your family. Such changes may include such things as marriage, the adoption of a child and the loss of alternative health insurance cover as the result of such things as the cessation of cover being provided through another family member because of death, legal separation, divorce, termination, retirement, reduction in working hours and similar changes.<br />
Almost all plans will also normally have a waiting period for membership which will typically be anywhere from 30 days to about 3 months. This waiting period must be applied consistently to all employees and during this time an employee is not covered by the group scheme.<br />
Where the group scheme which you are joining is being operated by an HMO then the HMO can also require a waiting period (often known as an affiliation period) where you will once again not be covered. HMO affiliation periods cannot normally be greater than 2 months and where a waiting period is required the HMO is not allowed to then impose any pre-existing condition exclusion periods.<br />
Under the provisions of Florida law any group <strong>health plan</strong> which provides dependent cover also has to provide cover automatically for newborns, newly adopted children and children who are placed for adoption for 31 days from the date of birth, adoption or placement. The can also require parents to register such children with the scheme within this 31 day period for cover to continue thereafter.<br />
For parents caring for disabled children who are covered under a group health insurance plan cover will generally continue beyond the age when a child would cease to qualify as a dependent, as long as the parents are able to show that the person in question cannot support himself because of mental or physical disability and that they are largely dependent upon the plan member for support.<br />
If you are working for an employer with at least 50 employees then you may take a leave of absence without losing you health insurance for up to 12 weeks in certain circumstances. This protection is guaranteed under the Family and Medical Leave Act (FMLA) to cover such things as childbirth, sickness or the need to take care of a seriously ill member of your family.<br />
Federal law allows states down to local government level to exempt government employees from specific coverage in self-insured group health plans and a lot of public employers in Florida take advantage of this to a greater or lesser degree. Because exemptions vary widely from one employer to the next it is wise to find out the precise coverage provided if you are a public employee. These details can also be found by contacting The Center for Medicare and Medicaid Services (CMS) which maintains a list of exemptions for individual employers.<br />
In spite of the fact that under Florida law you cannot be excluded from membership of a group health plan for reasons of health, there are some circumstances in which exclusion periods may be imposed for pre-existing conditions. However, this is a complicated area and one which is thus the subject of a separate article.<br />
